Job Summary:
Using client-centered strengths perspective, provide one on one supportive case management to residents based on trauma informed perspective

Essential Job Duties and Responsibilities: (Additional duties may be assigned)

  • Conduct phone screens and one on one-person intake interviews to determine applicant’s suitability for the program.
  • Establish program fee status, complete all intake paperwork including reference checks, fax admit form to main office. Include information about program fee (paid by DSS or self).
  • Maintain at least 20 residents in case load, and a maximum of 25.
  • Schedule move-in and be present to assist new resident. Ascertain she has adequate furnishings, bedding, working lamp and key to apartment. Make sure the checking list for the apartment is done and file.
  • Assist resident with identification of personal goals (ISP) and refer resident to the life skills training to develop life skills to foster independence. Assist resident in strategic planning to meet needs and attain goals. Provide educational information, referrals and linkage to community resources.
  • Meet formally with resident one (1) hour weekly or monthly, based on individualized service plan. Collect program fee if necessary. Make at least one home visit a month.
  • Read communication log daily to determine resident and compliance with curfew, medications, chore assignment, SI/SO policy and program participation. Address noncompliance.
  • Collect data and maintain all case records inclusive of verbal and written communication (progress notes). This progress notes should reflect the ISP. Compile, complete and submit accurate monthly statistics, including your monthly activity with clients and participation in the activity to your supervisor.
  • Schedule exit date, perform exit interview and apartment inventory with resident. Be present for move out. Return key to Front desk specialist. Complete documentation and fax departure form to main office.
  • Maintain contact with residents exiting the program for a minimum of six (6) months.
  • Attend workshops to increase knowledge base and job performance skills.
  • Represent YWCA at community and interagency meetings when requested by administration.
  • Provide weekly schedule to supervisor. Any change on it must be approved in advance.
  • Assist front desk specialist when needed.
  • Other duties requested by supervisor and/or Executive Director.

Team Member Responsibilities

  • Plan and execute informational, cultural, therapeutic, or recreational activities with residents including:
    • Coordinate one major event a year (Black History month (February), Women’s History day (March), Mother’s day (May), Hispanic Heritage Month (September), Halloween (October).
    • Develop and post flyers, memos, and signs, forms for activities in advance
    • Vigorously recruit participants
    • Seek resources within the community to assist in presenting activities
    • Compile statistical information
  • Coordinate the maintenance of the van:
    • Keep track of van’s mile sheet (i.e. dr’s appointments. shopping, field trips, etc.)
    • Maintain van (implementing clean up, oil change, making sure the van is in good standers).
  • Coordinate and execute monthly home meetings including:
  • Collect residents achievement data and complete award certificates for house meetings
  • Obtain supplies


Supervisory Responsibilities:

Minimum Qualification Standards:
Associate’s degree (A.A) or univalent from two-year college or technical school majoring in human services: and six months to one year related experience and/or training; or equivalent combination of education and experience.


Special Requirements:

  • Drivers License

Knowledge, Skills, and Abilities:


Ability to read, analyze, and interpret complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write speeches and articles using original or innovative techniques and styles. Ability to make effective and persuasive speeches and or presentation on controversial or complex topics to top management, public groups, and/or boards of directors.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions and percentages. Ability to apply concepts.

Reasoning Ability
Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems. Ability to deal with nonverbal symbolism (body language) in its most difficult phases. Ability to deal with a variety of abstract and concrete variables.
